A few months ago I helped an older patient sort out some follow up stuff after discharge and gave his daughter a number to reach me because there was some confusion about who they were supposed to contact. I used my Cloaked number instead of my actual cell thankfully because somehow that turned into her texting me every time they had a question. A significant amount of Philadelphia public schoolsā technology and other equipment goes missing each year due to poor inventory and tracking.
My poor lady tonight. She needs to go comfort care and pass, but family has made her Do Not Resuscitate - NO ESCALATION. So I am maxed out on her levophed (15 was the max per the order and family request). She is intubated and on a fentanyl drip. Towards the end of my shift, she spikes a 39.4 fever and becomes even more hypotensive (MAPs in the 30s). The only way to even sustain her BP is to cut her fent drip and turn it off. Despite her HR being 120, breathing over the vent at 40 BPM, Iām forced to completely take her fentanyl drip away to raise her BP on sheer pain. Pain is the only thing keeping her alive. Family refuses to go comfort care and let her pass because āJesus will take her when heās readyā, but sheās actively trying to die and now we are just making it painful for her. I donāt understand how the family can watch her die in pain and refuse to let her pass peacefully. Basically the title. This is in an international school in Sweden, 10th grade / high school. I have been substituting for a high school civics course, and the regular teacher recommended for me to show a documentary, specifically on former skinheads who were helping to now de-radicalize the youth. Basically it followed these men getting their old r*cist tattoos covered by an artist, and how they were able to re-build their life after prison, how they let go of hate, and now were in particular helping young men to not go down the same path. Anyway, the school had a problem with this. I got a really stern talking to after a few days about how the documentary was completely inappropriate, and how there's no redemption for some people, etc., and that I should've known better than to show something that implies that people who have formerly held radical beliefs can ever be forgiven and re-integrated into society. Hi, Iām a new grad in med surg tele for about 3 months now. I have been feeling overwhelmed and stressed about this job. On my days off all I think about it work. And I cry almost every shift on my way home. Iāve been clocking out late almost every shift as well trying to catch up on charting because dayshift is super busy and my time management is not the best. i notice this is the case mostly with male cnaās/hcaās. I routinely see them refuse help with 2pa/total care patients. Like, theyāll do the peri care and repositioning by themselves even if the patient is very heavy, and often even get them up onto a chair by themselves by just straight up lifting them from the shoulders in a hugging motion Usually, other cnaās offer them help and so do nurses, but they just absolutely refuse it. My first 10 years I always made seating charts, especially for my plan heavy classes. I havenāt the past 6, because I donāt know If their name in the system is going to cause an issue. I also canāt imagine having 25 or more 15 year olds standing in my room while I ask their names either and trying to get them in a spot. At the risk of sounding like a nagging and unexperienced RN, I would like to open up this thread to discuss the abuse of āseniorityā rank. Iāve been an RN for 3 years but Iāve been in my current position for 1 year. In general I like to think Iāve been taking taking positive/negative feedback well and been asking all the dumb questions for clarity and reached out to other RN on the floor for advice and what-not to compare notes and validate my thought process. I am not afraid to make mistakes as long as I am learning. At this one year mark Iāve felt personal growth and created trusting relations with my colleagues. With this growth, I can confirm what I assumed to be an abuse of power is in fact true. I watch new nurses come in and fail to ask for help because they are trying to prove themselves capable of doing the job or simply not knowing what they donāt know. I made my first real fuck-up at work after about a year and a half of working acute care, and I need someone to tell me Iām not alone. I was messaging a doctor about my patient and getting orders throughout the night. Everything seemed completely normal. Some students don't want to build a relationship with you beyond the traditional student-teacher relationship and THAT IS OK. I get so tired of 'build a relationship' being the response to seemingly any behavioral or academic issues. Some students don't want to share intimate details of their lives. It feels like a lot of admin want teachers to force things. This is my first year. I have 2 kids and a husband, so safe to say I have a lot going on outside of school. Iām incredibly overwhelmed. Iām incredibly stressed. Iām incredibly depressed.
