Patient Safety Organizations: Voluntary Relinquishment for the Cassatt Patient Safety Organization

The Patient Safety and Quality Improvement Final Rule (Patient Safety Rule) authorizes AHRQ, on behalf of the Secretary of HHS, to list as a patient safety organization (PSO) an entity that attests that it meets the statutory and regulatory requirements for listing. A PSO can be "delisted" by the Secretary if it is found to no longer meet the requirements of the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act) and Patient Safety Rule, such as when a PSO chooses to voluntarily relinquish its status as a PSO for any reason or when a PSO's listing expires. AHRQ accepted a notification of proposed voluntary relinquishment from the Cassatt Patient Safety Organization, PSO number P0136, of its status as a PSO and has delisted the PSO accordingly.

Advice for a new teacher (besides RUN)🏃‍♀️

Hello! I’m currently in the middle of getting my masters in elementary education. I am almost 50 and I was a stay-at-home mom until two years ago when I got a job as a one-on-one para for a little kiddo with CP. I have experience with kids, I have five of my own, but they never got into trouble at school. I had teachers constantly thanking me for how my kids behaved at school and I just brushed it aside with a thanks. Now I see why they thanked me! I’ve worked in kindergarten and now first grade, so my eyes are open to how children behave these days and still decided I wanted to teach - so I guess as long-winded as this was (sorry), I do have an idea of what I’m getting myself into.😬 I’m looking for advice on how to talk to parents about their child’s behavior or having difficult conversations about grades, etc. Any phrases you stick with or???? I really don’t know why I’m letting this brat get to me, especially since it’s summer and I know this kid isn’t going to be in my class next year. I had a student last year who refused to participate because they were not willing to search for a material they needed to do my class. I didn’t really have the time to search for material for them (and it’s not supposed to be my job anyway), so the kid just sat there. By the end of the class, I was telling the kid they were lazy since they also decided to disrupt my class a smidge, only for that kid after class was done to try and tell it to their homeroom teacher, in an effort to get me in trouble…. The next class I had with them a week later, they were trying to use it against me to be disruptive and display their attitude. For those moving to California

For the nurses moving to California from out of state for the amazing unions, conditions, and pay. Please do not keep voting red and for the very same policies that have kept nursing in other states working in unsafe conditions. From what I have heard, some nurses who land a job in Cali are very opposed to unions and keep voting red and against the very policies that California nurses have fought tooth to bone for. It was particularly concerning seeing the recent election results. If you vote for the same policies in your state, be prepared for California nurses to make below living wages and with unsafe ratios again :) California nurses have fought and spent their blood sweat and tears to ensure nurses here get adequate pay and most importantly, work in safe conditions. If you are moving to California, please vote for those very same policies. If nurses from out of state keep crossing the picket line, California could very well lose the very protections that makes nursing here great.

My former pupils wanted to send a letter to the principal to make sure I was their teacher next year

I was assigned as a substitute history teacher to a middle-school class that was deemed "troublesome". Yes, there are pupils that struggle to be in class. Yes, some struggle to fit in. And yes, some need more help than others. But they had been deemed a lost cause by some, and one teacher even refused to teach them. They felt this of course, and brought it up regularly. Yet, frankly, I loved having them. In (my country) 9th grade, we focus on democracy, and the big world wars, topics I'm deeply interested in. And they seemed to agree, and were active, questioning, and engaged in EVERY class I had. Yes, there was some noise, some who did not care, and some topics that did not resonate, but I simply COULD NOT SEE the troubled class the rest of my colleagues saw. We had fun, I got to be creative with my teaching, and they did SO great. First year teacher: Should I go for this job or wait for a k-5 classroom?

There is a private school, low pay, about a 2 minute drive from my house. I’m a newly single mama to a kindergartener & a 15 month old. I sure wish I would’ve gone to the kindergarten job interview at this school as I’m almost sure I would’ve gotten it. The principal keeps reaching out, we’re friends on facebook, and she keeps telling me about the jobs she has at her school. She now only has a 5-8 grade for Social Studies and ELA or a Full time Prek aide. I have only subbed for half the school year for 2nd grade in the district I student taught. They don’t have any openings this school year, I applied too late. The competition for my first year teaching job is unreal. I don’t want middle school, I am licensed K-6. Have you noticed a decline in caring for kids medical health?

When I went to school, I got lice and they basically told me to gtfo until I either shaved my head or got rid of them (which I completely understand!). The school nurse used to check us religiously and we also had eye/hearing exams. Now I’ve had students come to school with lice or ring worm or chicken pox?!?! Has anybody whos done private duty and/or home health been monitored the entire shift, every shift?

I took a PRN private duty assignment for a young adult pt who requires a lot of care. The pt lives with mom and grandma in an apt. They are always home, and the pts room is the living room and i can't tell if the grandma also lives in the living room, but she is there sitting by me every shift. She doesn't speak to me except if its to criticize how I did something. She also gets upset by random things, like for example, the apt is very cluttered and there is no table counter space anywhere and only one dining chair I can sit in the whole shift. I briefly placed a pill organizer on a chair that I hadn't ever sat in, and she got mad I did that, it was either the chair or a random stack of boxes or ontop of my open lap top which im sure wouldve also been a problem. I understand why that was an issue but I had to set it down somewhere quickly to go tend to the pt. I switched my shifts from 12 hrs to 8 hrs which did make a big difference, but its still hard because of the constant monitorin

Unpopular opinion: but I’m tired of AI

I am feeling incompetent & old bc I am so old school with note taking & studying but it’s still not enough to barely make the 75% mark. I feel like I cannot really “train” my AI to help me to study, I’ve seen success stories of people using AI, having it do their slides and break it down for them, create exams, concept maps etc. I spend hrs studying off it, creating questions hell I even have them do a podcasts & using their prompts & I still don’t have “it”. Then when you do the exam it’s not even similar to the actual exam .-., there’s too much inconsistencies within my program too- No ai on assignments- but also use Ai to make ur own exam- but don’t rely on it too much bc the info can be not what we are using for the exam. Most of our material is off the PowerPoint, & nursing school is not about memorizing - but how my brain works is I have to memorize the concepts in order to understand to apply any situation/concept. Best path for moving to NYC as a nurse?

Hi everyone, my long-term goal is to move to NYC, ideally in 2028. I currently live in Florida and I’m trying to decide between two nursing paths. Option 1: Start an ABSN program next summer, graduate in 2028, and apply directly to new grad residency programs in NYC or another major city. Option 2: Start a diploma RN program this fall, graduate in July 2027, work for one year in my hometown while completing an online RN-to-BSN program, then apply to jobs in NYC as an experienced RN. Which path would give me the best chance of landing a job in NYC? Is it generally easier to get hired through a new grad residency, or as an RN with a year of experience? If NYC doesn’t work out right away, I’d also be happy with another walkable city like Chicago, Boston, Philadelphia, or DC. Burnout

I feel it in my bones. I finally am out of survival mode in my person life, but work has been my personal hell. The old lady that takes her pills crushed takes little bitty bites and makes it worse for herself. I just sigh, and redirect. The person that refuses turns (A&Ox4), and has a self inflicted pressure injury makes me roll my eyes. The addict who complains about not having family members. I educated on cycles of addiction…..I’m empathetic, but I’m more stern with delivery. The person that says they can’t do their wound care, but is a walky talky and can use a mirror to do it….and also works in healthcare (also very simple wound care). The person that takes 15m to talk about random things while they can see the beads of sweat on my forehead and heavy eyes. I just have to literally walk out the door. The entitlement to take a bunch of food/drinks every day so you will have different snack at the shelter….when you are give food/shelter for practically nothing while recovering from

Hello! My work friend and I applied for the same position which is a step up from our current role (working with ALN) I wished her luck if she was applying and she was adamant she wasn’t interested and couldn’t meet the expectations for the role. It got to interview stage and we were both there. We are really good work friends and have been for over a year, we are the only two in our position and work really well together. I got the job, which means I’m her “boss” and I’m feeling really anxious of how our relationship will be. She’s told me not to change ANYTHING which unfortunately will happen and I have reassured her I’m working with her. Things seem strange between us but I think that’s just my anxiety of the situation. Is this profession slowly becoming less protective of teachers?

A few days ago I posted about an incident that happened on the second-to-last day of my final student teaching placement, and I honestly thought my teaching career was over before it had even begun. For anyone who didn’t see the original post, I accidentally collided with a child’s ankle while playing soccer with the kids during recess. From my perspective, I barely touched him, which is why I was so shocked when he said his ankle hurt. Afterwards, I also saw him hopping around on the same ankle, so I initially thought it was only a minor injury. The child’s parents came into the school, yelled at the principal and staff. They were mad - very mad . After hearing that, I genuinely thought, “Well, I’m completely fucked.” To make things harder, I’d also had a really difficult placement. Throughout the placement I was verbally abused by some of the kids, including racist comments directed towards me. When I brought those incidents up, I was basically told I just needed to move on. Sharing a win!

First things first, I work with students who have special educational needs. I have a student who is selectively mute. Let's call her Sarah.. It took Sarah over 6 months to talk to her female classmates. She doesn't talk to any of the boys and none of the adults at school. It was really hard to navigate how to deal with this. So each time I had questions for her, I just wrote them down. She alsno nodded yes or no. That's her main way of communicating. Yesterday however, another girl and Sarah walked up to me, and the other girl mentioned that Sarah really wanted to talk to me. So she tried, like 5 times and her lips were sealed, glued shut. I told Sarah there's no pressure and she could try any other time. Then all of a sudden, when there was no one else around she talked and said: "you're my favourite teacher". And after that she talked some more. Only started work 2 months ago, came back from maternity leave and was sick prior to that, so I've only known the students for two months.

Sending a condolence card to former HS teacher - okay to include personal updates, or not?

Former student looking for input here: I graduated high school in 2014. My senior year I had a teacher who I was very close to and who made a big impact on me. For the first few years after I graduated we stayed in touch pretty regularly through email and meeting up a few times a year. For the past several years though we've drifted out of contact, largely just due to things like the pandemic, my family moving away from that area, etc. I found out a couple weeks ago via Facebook that her husband recently passed away after a 3 year battle with cancer. (We're not connected on FB but I saw the obituary shared publicly). CCRN

Hello everyone, I’m taking my test in about a week and a half or so. I’ve been doing AACN practice exams and going over some videos and rationales when I’m done. I’ve been consistently scoring between 103-105/150 and I’m nervous that’s not in the “passing” range on the real exam. My weak points are endocrine and some cardiac. I’ve been using my free time to study but also been cramming while working full time. In those who have successfully passed and crammed, what scores were you getting if you took practice exams on AACN? Is getting 103-105/150 an indicator that I am on the right path to passing? My two lowest scores were on night shift at work with scores of 92 and 94 out of 150. I’m also using some videos on YouTube recommended to me by fellow colleagues that have taken it and used these videos to help them on the test. One of them failed and used these videos as a resource and passed the second time a couple of months ago. I do have the barrons book but honestly, I can’t sit there

Why do you think bedside nursing is so bad nowadays?

Hello, nurses. I’m a non-nurse but come from a relatively large nursing family. My mother is an NP and started in bedside in 2003. My grandmother worked as a nurse for 54 years and worked in either a hospital or surgery center the whole time (OR, post-op, ER, et cetera) from 1964 to 2018. My likely eventual sister-in-law is a new nurse as well, and I have some other relatives who have been in nursing since the 90s. While my relatives have stories dating back to the 1960s, I’ve noticed that the newer nurses I know seem to face these situations but amplified to an extreme due to lack of support, insane patients and family, overwork, and more. While every nurse in every timeframe faces insanity within their career, especially working bedside, it seems almost unmanageable now. Offer Rescinded

Context: I’m taking over a coaching role from last semesters coach that was fired for hazing students and is currently under investigation by TEA/CPS. Scheduled a contract signing 2 weeks ago, something came up and I couldn’t make it and had to reschedule; no problem. Rescheduled it for next week. Got a call from HR at the end of the work day a few days ago saying that they have to rescind the offer they made for me because they said I’m currently under investigation by CPS. I have a feeling that if I really was under investigation, I would have been legally informed by my school, TEA, or CPS right? I wouldn’t find out first from my new schools HR department and them tell me “we don’t know anything you’ll have to contact CPS.” I contacted CPS and they told me there was no case open for me. There’s no investigation notification on my teaching certificate. (There is on the old coaches certificate) I’m curious if anyone has ever run into a major technological mistake hurdle like this befo

What do you wish you’d done BEFORE students walked in?

First year as teacher of record (Middle School SDC) This fall will be my first year as the teacher of record in a middle school Special Day Class (grades 6-8), although I spent last year working in this same classroom as a long-term substitute. So I know the students, the school, and the routines fairly well, but this year the responsibility is officially mine. I’m spending the summer finishing prerequisite classes, reorganizing the SDC room, reading IEPs, planning schedules, creating classroom procedures, and trying to build systems that will make the year run smoothly. My question is: Looking back, what is something you wish you had set up before Day 1 that made a huge difference later in the year?
